Accessibility & Communication needs

In line with the NHS Accessible Information Standard, we want to ensure you can access our services easily.

  • Reasonable Adjustments: If you require a BSL interpreter, Easy Read letters or have any other communication needs, please tell us. We will add a Digital Flag to your record so we always meet your needs.
  • Alternative Formats: If you cannot use the online service, you can request a Large Print or Easy Read paper form from our reception desk at the Aldershot Centre for Health or Lower Farnham Road Surgery.

Change of Personal Details:

Please help us to keep our records straight by letting us know if you change your name, address or telephone number. Unless there are special circumstances, we regret we cannot keep on patients who move out of the practice area.

  • Complete our online change of details form to update your name, address, phone number, email address or date of birth.
  • Phone, email or visit the surgery.

Overseas visitors – who is eligible for NHS treatment?

If you are visiting from another country and need urgent medical attention we will treat you under the NHS system for a period of up to two weeks.  Please note that hospital care or other community care may incur a charge, depending on your country of residence.

Named Accountable GP

All patients at The Cambridge Practice have a named, accountable doctor who is responsible for coordinating their care.

Your named doctor will be allocated to you by the practice. You can still talk to or make appointments to see any other doctor, not just your named GP.

If you have a preference and would like to request a particular doctor at the practice to be your named GP, please contact us.
